Fill this in, print it, and give it to the pre-admission clinic — not on the day, and not only to the surgeon. Your anaesthetist is usually someone you meet for the first time an hour before.

Prescribed medicine — for my anaesthetic record

- Date of birth
- Procedure and date
- Medicine, exactly as printed on the pharmacy label
- How often I take it, and when I last did
- Prescriber name and contact
I am telling you this so it can be taken into account. I am not asking for advice about it here, and I have not changed how I take it without speaking to the person who prescribed it.
